Register Faith Formation

- Please complete the form on this page by September 4, 2026 to register for St. Michael Parish 2026-2027 Faith Formation programs (CGS, Spirit of Truth, Family of Faith).
  
  Please also complete the registration form for any child who needs to register for Sacramental Prep II for 2026-2027, regardless of whether you plan to attend faith formation programs, and even if your child receives faith formation via St. Michael School or other/homeschool curriculum.
- Your child should register for Sacramental Prep II if they need to receive communion and/or confirmation AND:
  
  were in Sacramental Prep I last year, or
  were in 1st grade at SMS, or
  was 1st grader or older attending regular faith formation programs offered by the parish, or
  was homeschooled using an age-appropriate faith formation curriculum. This is preparation that a child can take in addition to the other faith formation programs offered by the parish.

- If your child attends St. Michael School or is homeschooled, you are encouraged (but not required) to attend the other faith formation programs offered in the parish.
  
  However, if your child is preparing for the sacraments, your family needs to attend 4-6 of the Gathered Family Faith Formation sessions, as well as the parish sacramental retreats and rehearsals. Please register for Sacramental Prep II using the form below to stay updated as these sessions are scheduled.
